I can leave out a hair bow if it doesn't compromise the hair line, if that makes sense. But beyond minor changes, I am unable to alter designs. I, like you, am a consumer. I buy designs from awesome digitizers to be able to stitch out on finished products. Some digitizers wear a double hat and sell their designs as finished products, so they would be able to change designs.
